DEMOGRAPHIC DATA COLLECTION SOFTWARE – Custom intake survey to collect anonymous data, including options such as age, zip code, race, or gender. The data is stored in a CSV file that can be exported to Excel and easily extracted via the cloud or a USB thumb drive.
